陕北山区，四十八岁的拓泉，是方圆百里唯一的一个医生，拓泉家住在横山县南部贫困山区，这里也是革命老区，有名的陕北民歌《横山下来些游击队》就是当年发生在这里的故事。 拓泉家里有八口人，几十亩山地，庄稼的收成全靠天气，年景好的时候还可以过得去，年景不好时庄稼几乎无收，人们惧怕的不是贫穷，而是怕生病，怕看病，看不起病，唯一让老百姓欣慰的是他们这有拓泉这样的医生，生活中常常是拓泉正在地里做农活的时候就有人来叫看病，拓泉放下农活，骑上摩托车就走了，有时很晚才回来，有时到第二天、第三天才回来，碰到看不了的病，拓泉就陪着病人的家属亲戚把病人送到几十里外的县医院，甚至连生孩子有时没办法拓泉也得去，二十多年如一日，拓泉走遍了家乡的山山水水，不知道给多少人看好了病，也送走了多少年老而终的老人，对无可救药的病人，拓泉更像是尽职的牧师，让病人的家属在心里上能有安慰，这就是拓泉...。Running Time was filmed in black and white, in real time, and seemingly takes place in one continuous, fluid shot.
